Population Health Generalist - CMA/RMA or LPN - Full Time

The population health generalist is responsible for performing Pop Health-related duties on a professional level and works closely with senior Population Health management in supporting Care Management team members.  This position carries out responsibilities in the following functional areas: care team relations, training, data collection, assist with onboarding of new team members, provide support services to NOMS regarding EBO/EHR reports/workflows and  data sharing with organizational team. Experience with Diabetes preferred.

Essential Functions:

  • Assist with onboarding for all new Care Team members.
  • Assists in evaluation of reports, decisions, and results of department in relation to established goals. Recommends new approaches, policies, and procedures to effect continual improvements in the efficiency of the department and services performed.
  • Coordinates implementation/ maintenance of EMR, drives, Outlook and Software programs for team members.
  • Answers routine questions from team members about workflows and other Population Health related topics.
  • Assist with training on various topics for NOMS team members.
  • Oversight of Retina Vue cameras, schedules and trainings.
  • Performs other related duties as required and assigned.

Education and Work Experience:

  • Completion of an accredited Licensed Practical Nursing (LPN) program or Medical Assistant (Registered or Certified) program
  • Minimum of two years’ experience in health care setting preferred.
  • Experience with Diabetes preferred.
  • Progressively responsible experience in information systems.
  • Microsoft Office Application experience preferred but not required.


  • Unrestricted, Licensed Practical Nurse licensure by the State of Ohio or Certified/Registered Medical Assistant

Environmental/Working Conditions:

  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. Minimal exposure to communicable diseases.

Physical/Mental Demands:

  • Work is typically performed at a desk or table requiring the use of standard office equipment. Intermittent sitting, standing and stooping. Interaction with others is constant and can be interruptive.
